Dear sir/madam,

It is with great regret that I am writing this letter to you to say that I will no longer be able to continue on in my role as a dentist at Shrawley House Dental Practice.

My time at the practice has been brief, but I must stress that it is not down to the staff, management or my working practice that I am having to leave. Rather it is because of personal circumstances; my mother has become quite unwell over the last couple of months (having battled with cancer and kidney disease, the latter of which has gotten much worse) and after having discussions with my family we have come to the conclusion that I need to be there for her on a more frequent basis, as she needs the support from immediate rather than extended family. I've made this decision after a month and a half of careful and hard deliberation.

I am also reviewing my entire working life in light of this and may need to stop working for a period of time until my mother's circumstances change. 

I also wanted to apologise to you as this will come as a shock and will cause you more inconvenience. I understand that it is not pleasant having to change clinician often after having placed your trust in them; indeed I was hoping to be at the practice for many years. I wanted to reassure you that the staff and owners have been nothing short of fantastic, very understanding and supportive and will continue to be this way.

However I must put family first and so with a heavy heart say that I am leaving the practice at the end of September 2020.

I can assure you that you will be greatly cared for by the principal dentist (Agnes Bugaj) until the practice have found a new associate dentist to continue with your future care.

I apologise once again for any inconvenience that this may cause you in the short term, but I can assure you will be very well cared for under the staff at Shrawley House.
